MADISON – Parents and guardians of prospective kindergartners in Madison are beginning the registration process.

Student families can officially register between April 28 and May 9 at the Central Office.

“The end of the 2013-2014 school year is fast approaching, and it is time for us to prepare for next school year,” elementary coordinator Judy Warmath said. “These preparations include welcoming brand new kindergarten students to our system.”

“We’re always excited to meet fresh new faces and help them embark upon their scholastic careers,” Warmath said.

Children entering kindergarten for 2014-2015 must be five years old on or before Sept. 1, 2014. Registration forms are available at the district’s website at madisoncity.k12.al.us or at the Central Office, 211 Celtic Drive.

Parents must complete several forms before arriving to register: Parent Information Form, Parent Employment Survey, Student Registration Form and the optional School Lunch Program Questionnaire.

Parents and registering kindergartens can visit the Central Office from April 28 through May 9. Available hours are Monday-Thursday from 8:30 to 11:30 a.m. and 1 to 3 p.m. or Friday from 8:30 a.m. to 1 p.m.

Parents must bring photo identification, the Alabama Blue Immunization Form or certificate of exemption, copy of student’s birth certificate and Social Security card, custody papers for divorces and proof of residency, such a residential deed or rental lease and utility bill.

Parents will receive a list of the schools, including contact information for the principal, school address and phone number; school supplies list; 2014-15 school calendar, list of open house dates; and other school-specific details.

“We look forward to partnering with you on your child’s educational journey,” Warmath said.

For questions about registration, call registrar LaTwan Lynch at 256-464-8370, ext. 10366 or email to llynch@madisoncity.l12.al.us.
